Purelements After School Application
The parent/guardian will have to download the application, open it in Adobe and they will be able to fill in the document on the computer. Please find and click on "Save As" a new document, preferably with your child's name and grade (Ie: Aisha-Darby-6) and email the new document so we can start to input them into the database.
